Acute viral bronchiolitis is the most common lower respiratory tract infection in infants under 2 years of age. There is currently no effective treatment and supportive therapy remains the standard treatment. Airway edema (abnormal accumulation of fluid) and mucus plugging can cause shortness of breath and difficulty breathing in these patients. Spraying hypertonic saline through an inhaler (nebulizer) can be effective in treating acute bronchiolitis because it can improve respiratory hygiene. This review was conducted to evaluate the effects of inhaler (nebulizer) administration of hypertonic (≥ 3%) saline compared with nebulization of saline (0.9%) in children with acute bronchiolitis. Determining the therapeutic value of hypertonic saline may provide an inexpensive and effective treatment for these patients. A meta-analysis suggests that nebulization of hypertonic saline can reduce the average length of hospital stay between 1.2 days in hospital for mild acute bronchiolitis and improve clinical severity in both outpatient and inpatient groups. Considering the clinically significant benefit and good safety profile, the use of nebulized hypertonic saline solution 3% in combination with bronchodilators is considered an effective and safe treatment for children with mild and moderate acute viral bronchitis.
